JMS Trust: Person-centred supported living
The Trust final piece of work has been creating a resource called “Our journey, their future: Person-centred supported living” to share the learning and reflections of the families we’ve worked with, in the hopes of helping other families who have been thinking about doing something similar.
JMS Trust has supported on 17 projects, impacting 64 families and 70 people with learning disabilities, where people now have a home that is bespoke to them.

Who is eligable for support?
Supported living is available to adults aged 18+ who need support due to a disability, mental health condition or additional needs. Support is personalised and based on an assessment of individual needs.
How are the support plans put together, and are we involved?
Each supported person has an individualised support plan developed in collaboration with themselves, their family members and the relevant wider community. The supported persons support plan will be created around the individuals goals, aspirations and preferences.
What is we have a concern or complaint?
We take all concerns seriously and have a clear process for raising and resolving them. You can also speak with the service and registered manager at anytime.
Are you CQC registered?
1st Enable are fully regulated by the CQC. Our CQC ratings reflect our commitment to delivering safe, high quality and person-centred support. You can view the report here.
