Our support focuses on what is important to the individual, whether that is at home, at work or in the community. It centres around the things they want to do in life and what is important to them. By listening to what people say through their word or behaviour, we ensure that our support matches their needs. Our staff are supported by national specialists in best practice, ensuring that expert advice is always on hand
Active Support & Audits
Our active support approach helps us support people in a way that enables them to participate in all aspects of their life, allowing them to be as independent as possible.
We have a comprehensive range of policies and regularly audit our support services. We have also signed up to the driving up quality code and involve the people we support in checking our services, through our checker program. We aim to learn from our mistakes and make sure our support continually improves
Our Person-Centred Approach
Person centred means putting the individual at the heart of everything we do. Our approach focuses on safety, choice and independence, supporting people to achieve their goals and live meaningful lives.
Getting To Know The Individual
We take time to understand each person’s preferences, needs, strengths and aspirations. Using person-centred tools and planning approaches, we build tailored support plans around what matters most to the individual, helping them feel safe, valued and respected.
Quality That You Can Trust
Our staff are supported by national specialists and guided by robust policies and quality frameworks. We audit our services regularly, follow the Driving Up Quality Code, and involve the people we support in service checks. We also operate a clear complaints process to ensure learning and continuous improvement.
Support That Will Grow
Support is reviewed regularly and evolves as people’s needs and goals change. Through ongoing monitoring, feedback and active support practices. We help individuals develop skills, confidence and independence in everyday life, at home and in the community.
Outreach
Our Outreach service provides the highest quality care and support possible at home and in the community for people with mild, moderate and severe learning disabilities. We believe in providing a personalised service and reflecting this in our support packages. These package are always individually tailored to meet the specific needs of the person. Our overall aim is to support people to reach their fully potential and we achieve this by facilitating the learning of new skills and confidence building, whilst embracing the value of learning.
Our outreach service provides a range of support to help people with a learning disability to live independently and maintain their tenancy. At the same time, we work towards supporting people to achieve their short and long term goals and aspirations. Support is structured and covers various life skills including:
Managing health and well-being
Accessing work or training opportunities
Accessing local community groups
We will work closely with the people in outreach, the people who are important to them and their social workers to develop a person centred support plan that goes into more detail about what they want to achieve. We create an action plan which provides details of what support these individuals may benefit from and when the best time to work with them is. The important thing to know is that when we are working with/supporting individuals in outreach is that we work on their schedule, not ours. As well as providing direct practical support to people in their own homes, we forge ties with community groups, charities, social and leisure groups, educational providers and employers to help make the most of the opportunities on their doors step
